A study on the community long-term care recognized by service stakeholders of Korea and Japan - a comparative case study between ¡®Changwon-si¡¯ and ¡®Echizen-si¡¯ -

Abstract
The purpose of this paper is to examine ¡®commnunity long-term care¡¯ recogonized by service stakeholders through the comparison between ¡®Changwon-si¡¯ in Korea and ¡®Echizen-si¡¯ in Japan. Though many recent studies have focused on the comparative LTC policies among developed countries at a national level, few studies have focused on the comparison of care delivery systems at a community level. This study aims at understanding the relationships between the care policy for elders and the recognition of care by community care-stakeholders in both cities. To do so this study has collected qualitative data from the in-depth interviews of service stakeholders in both Changwon and Echizen. The interview data have been analyzed by a research method of directed content analysis within the analytic frame of the Gilbert¡¯s welfare service delivery system. The results of this study show the three core points. At first while ¡®care management¡¯ and ¡®community care meetings¡¯ in Echizen play key roles for meeting elders¡¯ care needs for living in a community, Changwon community has rarely chances of selecting an adequate service type and reflecting elders¡¯ decions for their care. In addition the linkage among care-stakeholders in Changwon was also turned out to be very restricted by many barriers from the strict information exchange policy and bureaucracy of inner organizations. Secondly in terms of accessibility, while there are not so many types of care services in Changwon, more types of services including respite care or small-size multi-purpose centers in Echizen are available but with some restrictions of co-residing caregivers¡¯ conditions. Finally regarding care responsibility a community has located on a vague position between an individual and a nation in both cities. Based on these results the Korean policy of community care should provide a community with the key role of care coordination rather than just a ¡®place¡¯ role of care delivery. The implication of this study is believed to compare the reality of community care between both communities in Korea and Japan.
